Measure Overview Medicaid Goal

This measure reports the percentage of patients whose provider used Prime Suite to share electronic access to at least one education resource during the required time period.

Setup Requirements

Electronic access to education materials is provided as a link in the Clinical Visit Summary. When the Clinical Visit Summary is made available on the patient portal, the patient then has access to the educational materials. This means patients must be registered for the patient portal during the required time period to qualify. This is indicated by the Patient Portal flag.

You must have purchased and implemented the Greenway Patient Portal before patients can access their information on the patient portal. If you would like to set up the patient portal functionality, please contact your sales representative.

Denominator

The number of unique patients who had at least one non-voided face-to-face visit with an eligible provider or clinician checked-in during the measurement period.

The measurement period is set in Prime Suite Reporting 2020.

Numerator

Patients where all the following occurred within the calendar year that the measurement period begins in:

  • The patient was registered for the patient portal indicated by the Patient Portal flag.
  • Electronic access to education materials related to the patient's care was supplied to the patient by selecting an InfoButton for one of the patient's medications or problems. Only Info buttons in the following sections will qualify for this measure:
  • Problem List in Facesheet
  • Assessment Section in a Note
  • Medication List in Facesheet
  • Medications in the Plan section of a Note
  • The Electronically Identified Patient Education Materials Provided Electronically check box was selected in the Plan section of the visit's Note.
  • The Clinical Visit Summary for the visit was made available through the patient portal.
  • A visit with the provider was linked to a note documenting that patient-specific education was provided.
