Important Note About Patient Portals

This documentation includes workflow information for sites using the Greenway Patient Portal. Sites using the Medfusion patient portal should read the following topic: Medfusion Portal Notes. Refer to the help documentation provided by Medfusion for any other Medfusion-related workflows.

Measure Overview

This measure reports the percentage of patients whose provider used Prime Suite to share electronic access to at least one education resource during the required time period.

Measure Overview
Denominator The number of unique patients who had at least one non-voided face-to-face visit with an eligible provider or clinician checked-in during the measurement period.
Numerator All patients registered for the patient portal who were supplied electronic access to patient-specific education clinically relevant to the patient’s care during the required time period.

Denominator Criteria

The number of unique patients who had at least one non-voided face-to-face visit with an eligible provider or clinician during the measurement period. This visit must have been checked-in during the measurement period. Additionally, the best practice is to also check-out the visit during the measurement period.

Numerator Criteria

The numerator action must be completed during the calendar year that the measurement period begins in. For example, if the measurement period begins on 3/3/2020, the numerator must be met during the period that begins on January 1, 2020 and ends on December 31, 2020.

All the following are required to qualify for the numerator:

  • The patient is registered for the patient portal indicated by the Patient Portal flag.
  • The InfoButton for a medication or problem is selected in either the Medication List or Problem List on the Facesheet, Assessment section of a Note, or Medications in the Plan section of a Note.
  • The Electronically Identified Patient Education Materials Provided Electronically check box is selected in the Plan section of the visit’s note.
  • Electronic access to education materials is supplied to the patient during the required time period.

The Prenatal flowsheet does not qualify as patient education.

  • These education materials must be related to the patient’s care.
  • Since this is a provider-specific measure, a visit with the provider must be linked to a note documenting that patient-specific education was provided.

Setup Requirements

Electronic access to education materials is provided as a link in the Clinical Visit Summary. When the Clinical Visit Summary is made available on the patient portal, the patient then has access to the educational materials. However, this means patients must be registered for the patient portal during the required time period to qualify. This is indicated by the Patient Portal flag.

You must have purchased and implemented the Greenway Patient Portal before patients can access their information on the patient portal. If you would like to set up the patient portal functionality, please contact your sales representative.

To determine whether a patient has registered for the Patient Portal:

1. Search for and select the patient.
2. Review the icons shown for the patient or select the Patient Flags icon to open the Patient Flags dialog box.
3. Scroll down in the General list to see the flags related to electronic access. Selected flags are displayed in bold.
  • Patient Portal Invite: The patient has been invited to the Patient Portal; also used when the patient declines the invitation.
  • Patient Portal: The patient has registered for the Patient Portal. This flag is set automatically when the patient registers and cannot be changed manually.

The patient must have the Patient Portal flag to qualify for this measure.

Patient-Specific Education Workflow

In Prime Suite, you provide patient-specific education electronically through the Greenway Patient Portal. You can provide the materials using Infobuttons. However, only Infobuttons available in the following places qualify patients for this measure.

  • Medication List on the Patient Facesheet
  • Problem List on the Patient Facesheet
  • Assessment Section of a Note
  • Medications in the Plan Section of a Note

When using an Infobutton on the Facesheet, the visit must be linked so that the education material is linked to a visit and included in the Clinical Visit Summary for that visit.

To electronically supply patient-specific education documents:

1. Access any of the following for the patient:
  • Medication List on the Patient Facesheet
  • Problem List on the Patient Facesheet
  • Assessment Section of a Note
  • Medications in the Plan Section of a Note

Only Infobuttons in these sections qualify for this measure.

2. Select an Infobutton next to a problem or medication.

The National Institute of Health’s MedLinePlus service displays in your workstation’s default Internet browser with information about the medication or problem. The patient receives a link to this material.

The date you select the Infobutton is used as the date you generated the educational materials. This must occur during the calendar year to qualify.

3. Select the Electronically Identified Patient Education Materials Provided Electronically check box in the Plan section of the note created for the visit.

To qualify, this note must be in a signed state. The date of the last Save & Sign action must occur during the calendar year. If this note is amended, it must be re-signed and saved during the calendar year or the patient will not qualify. While the note is being amended, the patient will be removed from the numerator.

4. Ensure the Patient Portal flag is set on the patient’s Facesheet. The patient must have electronic access to their health information to qualify.
5. Make the visit’s Clinical Visit Summary available to the patient via the patient portal during the calendar year.

If the patient’s preferred communication method is set to Patient Portal, the Clinical Visit summary is available on the portal as soon as the note is signed. Otherwise, after the note is signed, use one of the following methods to make it available.

From a Note:

a. Select the View Clinical Summary link under Note Options in the left navigation.
b. Select the Available in Portal check box at the bottom of the Clinical Visit Summary.
c. Sign the Note.
d. Select Save.

From the Superbill Summary:

a. Access the Superbill Summary for the patient.
b. Select the Clinical Visit Summary link.
c. Select the Available in Portal check box at the bottom of the Clinical Visit Summary.
d. Select Save.

From the Facesheet:

a. Open the patient's Facesheet and scroll to the Visit History section.
b. Select the link for the visit to open the Clinical Visit Summary. You can access the patient’s last five visits.
c. Select the Available in Portal check box.
d. Select Save.
